This is Emerson K55HXNTG-4880 Fan Motor Lennox 72H3601 1/4HP 825 RPM 208-230V  used part.
This part was removed by HVAC installer from a working unit.
Old system was replaced with newer one (more energy efficient) and was in good working condition.
